Dear Friends,

We are the participants of the Broadway Connection--Mount Pleasant Adult Day Program. We would like to let you know about our program and the benefits we have gained from being part of it. If, after you have read this letter, you would like more information or would like to learn how you can become part of our group, you can contact our Program Staff.

"Broadway Connections is made up of disabled people who are able to come together as a group. The members are able to do many things which are very unique."

"We get together once or twice a week, on Tuesdays and Thursdays."

"I became part of this group through The Long Term Care worker who works within the Community Health Centre."

"For many of us our lifestyles changed dramatically,  sometimes at a rapid rate, and that is frightening at times.  The group is a place which is firm and secure. When we're in trouble, the support comes from within the group."

WHAT IS BROADWAY CONNECTIONS?

Broadway Connections is a program designed for young and mid-aged adults with physical disabilities, residing in Vancouver.

Our overall objective is to increase the independent living skills and quality of life of each participant, through:

. peer support groups
.  vocational awareness
.  life skills
.  wellness programs
.  recreation and exercise
.  group discussions


.  social programs
.  outings
.  educational programs
.  entertainment
.  self-advocacy


Several factors contribute to making Broadway Connections an exciting, caring group:

.  the contribution of the group members to the direction and the content of the programs.
.  the rich diversity of background, skills, interests, physical challenge and ethnic culture of participants and staff.
.  the Neighbourhood House setting, which provides a warm, casual community ambience as well as opportunity for participants to access other programs, services and special community events.


WHERE IS BROADWAY CONNECTIONS?

We Meet At:

Mount Pleasant Neighbourhood House
800 East Broadway, Vancouver

Tuesdays and Thursdays
From 3:00 p.m. to 7:00 p.m.

 


WHAT DOES BROADWAY CONNECTIONS COST?

Transportation: Most of our participants rely on the services of HandyDART for transportation at the current cost of $2.00 per trip. For outings, our staff make arrangements through HandyDART for transportation to and from the destination.

Meals: A hot dinner is provided at a cost of $3.00


Participation may be funded through Vancouver Coastal Health--Continuing Care Division.

Interested potential participants must be assessed  by a Long Term Care Case Manager for eligibility.

We have a limited number of spaces for private pay participants as well.
