Referral to the program can be made on your behalf, by a family member, a friend or a health care professional directly to the Home Health program. Simply call : Home Health's Central Intake Number 604 953-4965.

If you do not qualify under Fraser Health's  Home Health program, you can purchase service privately. Please speak to the Administration or Nurse to arrange private payment for the program.
